THS Sport Physicals for the 2005-2006 School Year
Wednesday, May 11th 6:00-8:00pm in Aux Gym at Terre Haute South $10.00
*Parents must sign physical form before the doctor will check the athlete. Physical Form online (pdf). Paper forms are available in THS Athletic Department, Main Office, or Middle Schools. Only offered once for this next school year so do not wait!
